Job Title:  ACT Registered Nurse ( Per Diem)      

Location:   ACT Strive Team 33 Church Street, 3rd floor Fredonia, N.Y.14063.       



The ACT Strive  R.N provides initial and ongoing assessments of clients, including client history, evaluation of problems, and formulation of diagnosis.  Responsible for a variety of medical and psychiatric functions including: coordination of community-based health care, psychiatric assessment, counseling interventions, crisis intervention, psychotropic medication administration and monitoring.  Develops treatment plans and offers individual, family and group counseling and psychotherapy to assist consumers in achieving goals. 

Duties and Responsibilities:

  • Provides program consultation and education to community members.
  • Documents consumer progress per established guidelines.
  • Trains staff in medical treatment interventions.
  • Participates in daily Morning Meetings.
  • Participates in 24-hour, on call rotation.
  • Routinely conducts lethality and mental status assessments.
  • Provides nursing-related services in the community and the consumer's own milieu.
  • Oversees blood screenings when determined necessary, and monitors for therapeutic ranges.
  • Attends to the annual health screen and makes recommendations to the specific team member(s) to ensure follow-up.
  • Maintains the medication room.
  • Prepares and dispenses meds daily.
  • Provides injectable medications to consumers outside of the clinic environment.
  • Coordinates all pharmacy, psychiatrist and consumer medication needs.
  • Case management duties.
  • Willing to take on many non-nursing clinical tasks (i.e., case management, counseling, etc.)
  • Demonstrates good listening skills with the ability to accept a consumer where he or she is at without imposing values.
  • Able to work in any type of environment.
  • Maintains good communication with inpatient facilities to insure appropriate discharge planning for consumers' medical needs.
  • Possesses strong medical nursing skills as well as psychiatric nursing skills.
  • Demonstrates a good working knowledge of both medical and psychiatric pharmacology.
  • Able to recognize and manage client crises.

Minimum Qualifications:

The ACT R.N. has a bachelor's degree in Nursing and relevant credentials (i.e., R.N.) and a minimum of two years' experience as a professional nurse with at least one year's experience working within a mental health setting which includes providing direct services to severely and persistently mentally-ill individuals, and possesses a valid NYS driver's license.
