Job Title: Senior Care Manager Supervisor

Reports To: Program Director

FLSA Status: Exempt



SUMMARY OF DUTIES:

Provides supervision and guidance to Health Home Care Managers as well assisting the Program Director in program operation. Provides direct care management to Health Home members.

RESPONSIBILITIES:

  • Using Netsmart assigns new cases to care managers according to care manager skills and members need.
  • Provides supervision to assigned staff.
  • Provides direct care to members as reviewed in the care manager description: Intake and Screening, Assessment, Care Management Planning and Coordination, Implementation of Care Management Plan, Monitoring and Follow up, and Linkage/Exit Planning.
  • Reviews and assists staff in completing the above responsibilities while assuring completion of needed services in a timely manner.
  • Conducts random reviews in Netsmart to determine completeness of care and offers support as needed.
  • Assists staff with linkages to community resources.
  • Conducts weekly staff meetings with staff in the absence of the Program Director.
  • Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.

QUALIFICATIONS:

  • A master's degree in a behavioral health related field.

OR

  • A bachelor's degree in Social Work or a related field and 3 years case management experience in providing direct services to behavioral health consumers or in linking individuals with behavioral health, substance abuse or medical diagnosis to a broad range of services essential to successfully living in a community setting.

AND

  • A valid NYS drivers' license.
  • Automobile in good working order.
  • Must have Accredited NYS Defensive driving course; or obtain within 15 days of hire.
  • Must maintain Active NYS Defensive Driving class every 3 years, upon expiration.
  • If driving one's own vehicle, must maintain valid inspection and automobile insurance.
  • Must provide proof of insurance upon hire and when requested.
