Case Manager of ACT Strive


·  Coordination and primary implementation of case management activities.

·  Consults with the Program Director and psychiatrist regarding response to treatment interventions.

·  Provides activities of daily living skills training and mobility training

·  Provides money management and budgeting training.

·  Participates in Morning Meetings.

·  Provides program consultation and education to community members.

·  Acts as a liaison where a brokered, specialty service may be required.

·  Documents recipient progress as per established guidelines.

·  Responsible for the management of client charts.

·  Develops initial and on-going treatment plans.

·  When indicated, transport clients to scheduled activities.

·  Work with clients in their own natural environment.

·  Assist clients in accessing supports to maintain community stability.

·  Adhere to person-centered planning philosophy and service provision.

·  Access benefits: Accompany clients to medical and financial appointments.

·  Accompany clients to scheduled court appearances.

·  Works with minimal supervision in the field.

·  Routinely conducts lethality and mental status assessments.

·  Participates in 24-hour, on-call rotation.

·  Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.


·  Has working knowledge of community resources.

·  Able to recognize and manage client crises.

·  Demonstrates creativity in service delivery and planning.


·  A bachelor's degree in a human service field and at least two years' experience as a case manager, working directly with severely and persistently mentally-ill individuals.


·  A valid NYS drivers' license.

·  Automobile in good working order.

·  Must have Accredited NYS Defensive driving course; or obtain within 15 days of hire.

·  Must maintain Active NYS Defensive Driving class every 3 years, upon expiration.

·  If driving one's own vehicle, must maintain valid inspection and automobile insurance.

·  Must provide proof of insurance upon hire and when requested.
