Transitional Specialist of the Lighthouse

RESPONSIBILITIES:

  • Conducts an assessment of each resident.
  • Assists client with obtaining GED prior to completing Lighthouse through referral, linkage, and follow-up
  • Assists with linkage to volunteering and/or education
  • Assists with linkage to independent/supportive housing upon program completion
  • Assists with linkage to services upon program completion to include health home linkage, eligible benefits
  • Consults with the treatment team to ensure a holistic and integrated planning process.
  • Conducts group sessions on a variety of skill building topics.
  • Teaches job-seeking and job-retention skills.
  • Manages Lighthouse store. 
  • Assist client to obtain wardrobe appropriate for vocational activity.
  • Respond to client concerns regarding impact of employment on benefit status.
  • Maintains an updated file of community resources.
  • Participates in client aftercare planning to ensure availability of needed supports.
  • Arranges for guest speakers in areas of interest to residents.
  • Participates in 24-hour, on-call rotation. 
  • Provides coverage for case manager
  • Participates in house management activities as assigned by supervisor.
  • Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.

REQUIRED KNOWLEDGE AND SKILLS:

  • Must have knowledge of case management principles, substance abuse treatment, and vocational planning techniques.
  • Must be goal directed, organized and able to work independently. 
  • Demonstrated ability to relate in a supportive manner to recovering women from diverse backgrounds required.
  • Spanish speaking desirable, but not required.

QUALIFICATIONS:

  • Bachelor degree in social work, psychology or closely related human services field, Plus demonstrated experience or training in vocational planning services. 

 OR

  • Associate degree in a relevant field, Plus two years of full-time paid experience in a related mental health, substance abuse or social service agency.

AND

  • A valid NYS drivers' license.
  • Automobile in good working order.
  • Must have Accredited NYS Defensive driving course; or obtain within 15 days of hire.
  • Must maintain Active NYS Defensive Driving class every 3 years, upon expiration.
  • If driving one's own vehicle, must maintain valid inspection and automobile insurance.
  • Must provide proof of insurance upon hire and when requested.
