Addiction Counselor of RASI

FLSA Status: Exempt

Salary: $43,500.00 - $66,242.27 per year

SUMMARY OF DUTIES

The Addiction Counselor is responsible for the primary case management of an assigned caseload.  The Addiction Counselor performs all counseling duties, including individual and group therapy, client advocacy, and discharge planning. 

RESPONSIBILITIES

  • Develops initial and comprehensive recovery care plans based on preliminary assessments.
  • Monitors and revises recovery care plans on a monthly basis, with input from patient and/or multi-disciplinary treatment team.
  • Develops discharge and aftercare plans including identification of potential problems and linkage to other appropriate treatment providers, based on individual client need.
  • Meets individually with patients on assigned caseload, a minimum of two times per week, to develop and/or revise recovery care plans.
  • Meets with patients and their families bi-monthly to address family issues.
  • Contacts referral sources, family members and significant others to encourage their participation in planning and implementation of treatment.
  • Documents all individual sessions and collateral contact in patient files on a weekly basis, in order to measure patient progress in treatment.
  • Participates in the development and implementation of appropriate discharge and aftercare plans for patients on assigned caseload.
  • Maintains required service, statistical and administrative records.
  • Participates in staff meetings, case conferences and in-service training.
  • Facilitates group therapy.
  • Provides educational lectures on various chemical dependency related topics, and basic living skills.
  • Assists in client transportation as assigned.
  • Perform all other duties as assigned

QUALIFICATIONS

  • Qualified Health Professional:  CASAC/CASAC-T
  • Associate degree in human services field PLUS 2 years full-time paid direct care experience in a chemical dependency program including experience-providing services to adolescents.
  • Valid NYS Driver's License
  • Must have accredited NYS Defensive Driving course; or obtain within 15 days of hire
  • Must maintain active NYS Defensive Driving course every 3 years, upon expiration
