Transitional Specialist of Lighthouse Women's Residence

Located at 244 Hempstead Ave 35 Hours Full Time, Hourly 

FLSA Status: Non-Exempt 

Starting Rate: $18.50 per hour 

SUMMARY OF DUTIES:

The Transitional Specialist is the member of the multidisciplinary team who assumes primary responsibility for assisting resident in development of housing plans and to identify educational/vocational resources in the community.  Works in collaboration with Case Manager to ensure that residents obtain needed off site support services.  Helps clients determine their eligibility for social services and other benefits and assists in the application process when needed.  Teaches daily living skills to clients.

RESPONSIBILITIES:

·  Conducts an assessment of each resident.

·  Assists client with obtaining GED prior to completing Lighthouse through referral, linkage, and follow-up

·  Assists with linkage to volunteering and/or education

·  Assists with linkage to independent/supportive housing upon program completion

·  Assists with linkage to services upon program completion to include health home linkage, eligible benefits

·  Consults with the treatment team to ensure a holistic and integrated planning process.

·  Conducts group sessions on a variety of skill building topics.

·  Teaches job-seeking and job-retention skills.

·  Manages Lighthouse store. 

·  Assist client to obtain wardrobe appropriate for vocational activity.

·  Respond to client concerns regarding impact of employment on benefit status.

·  Maintains an updated file of community resources.

·  Participates in client aftercare planning to ensure availability of needed supports.

·  Arranges for guest speakers in areas of interest to residents.

·  Participates in 24-hour, on-call rotation. 

·  Provides coverage for case manager

·  Participates in house management activities as assigned by supervisor.

·  Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.

REQUIRED KNOWLEDGE AND SKILLS:

·  Must have knowledge of case management principles, substance abuse treatment, and vocational planning techniques.

·   Must be goal directed, organized and able to work independently. 

·  Demonstrated ability to relate in a supportive manner to recovering women from diverse backgrounds required.

·  Spanish speaking desirable, but not required.

QUALIFICATIONS:

·  Bachelor degree in social work, psychology or closely related human services field, Plus demonstrated experience or training in vocational planning services. 

 OR

·  Associate degree in a relevant field, Plus two years of full-time paid experience in a related mental health, substance abuse or social service agency.

AND

·  A valid NYS drivers' license.

·  Automobile in good working order.

·  Must have Accredited NYS Defensive driving course; or obtain within 15 days of hire.

·  Must maintain Active NYS Defensive Driving class every 3 years, upon expiration.

·  If driving one's own vehicle, must maintain valid inspection and automobile insurance.

·  Must provide proof of insurance upon hire and when requested.
