Team Leader/Harambee House

FLSA Status: Non-Exempt

Pay rate: $27.47

SUMMARY OF DUTIES:

Orients new staff to program mission, goals policies and procedures; monitors and evaluates staff performance, provides direct supervision to on-site staff. identifies problems related to resources and personnel management; assists House Manager with internal and external reports to ensure compliance with funding requirements. Provide full case management services to all Harambee House clients. Obtain linkage and advocacy services in areas such as, financial entitlements, legal services and medical care. Maintains documentation necessary for client service and performs record keeping in accordance with agency, county and federal funding requirements. Maintains a fundamental understanding of mental illness and substance use disorders; their related symptomology and rehabilitation. Must be able to develop positive collaborations in the neighborhood. Treats all residents and staff with dignity and respect, and requires staff to do the same. Other duties as assigned.

RESPONSIBILITIES:

  • Initiates and maintains effective working relationships with all systems involved in providing services to the homeless
  • Advocates for the unique service needs of chronic homeless individuals who possess no linkage to treatment and are maintain none compliance with all systems of care
  • Assesses client's capacity for independent community living, formulates individual service plans, links and monitors client participation in needed services
  • Works with client to develop and enhance independent living skills and effective interpersonal communications, in order to achieve client's goals and objectives.
  • Works to develop and maintain effective formal and informal support networks including family, significant others and service providers.
  • Intervenes in crisis or pre-crisis situations to limit and contain problems
  • Transports clients to appointments, and provide advocacy, when needed.
  • May work with individuals harmful to themselves or others
  • Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.

REQUIRED KNOWLEDGE AND SKILLS:

  • Requires knowledge of mental illness diagnosis and symptomatology.
  • Possesses familiarity with community resources, including entitlement programs, medical, financial, legal services, supported housing and section 8 programs, mental health and addiction services.
  • Able to communicate clearly in written and spoken English, fluency in Spanish preferred.
  • Ability to engage with chronic homeless population a MUST.

QUALIFICATIONS:

Master's degree in a human service field, AND one year of paid work experience in a mental health setting or supervised professional internship in a mental health setting

OR

Bachelors degree in a related field plus two (2) years full time paid service experience in an approved mental health or social service agency

OR

Associates degree or High School diploma and (5) five years paid experience in an approved mental health or social service agency.

AND

  • A valid NYS drivers' license.
  • Automobile in good working order.
  • Must have Accredited NYS Defensive driving course; or obtain within 15 days of hire.
  • Must maintain Active NYS Defensive Driving class every 3 years, upon expiration.
  • If driving one's own vehicle, must maintain valid inspection and automobile insurance.
  • Must provide proof of insurance upon hire and when requested.
  • Bachelor's degree in social work/related field and two years full time paid experience operating a residential facility

OR

  • Associates degree or equivalent in human service field and five years full time paid experience operating a residential facility

AND

  • A valid NYS drivers' license.
