SUMMARY OF DUTIES:
Provides supervision and guidance to Health Home Care Managers as well assisting the Program Director in program operation. Provides direct care management to Health Home members.
RESPONSIBILITIES:
Using Netsmart assigns new cases to care managers according to care manager skills and members need.
Provides supervision to assigned staff.
Provides direct care to members as reviewed in the care manager description: Intake and Screening, Assessment, Care Management Planning and Coordination, Implementation of Care Management Plan, Monitoring and Follow up, and Linkage/Exit Planning.
Reviews and assists staff in completing the above responsibilities while assuring completion of needed services in a timely manner.
Conducts random reviews in Netsmart to determine completeness of care and offers support as needed.
Assists staff with linkages to community resources.
Conducts weekly staff meetings with staff in the absence of the Program Director.
Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.
QUALIFICATIONS:
A master's degree in a behavioral health related field.
OR
A bachelor's degree in Social Work or a related field and 3 years case management experience in providing direct services to behavioral health consumers or in linking individuals with behavioral health, substance abuse or medical diagnosis to a broad range of services essential to successfully living in a community setting.
AND
A valid NYS drivers' license.
Automobile in good working order.
Must have Accredited NYS Defensive driving course; or obtain within 15 days of hire.
Must maintain Active NYS Defensive Driving class every 3 years, upon expiration.
If driving one's own vehicle, must maintain valid inspection and automobile insurance.
Must provide proof of insurance upon hire.
