ACT (Assertive Community Treatment) is an integrated evidence-based treatment,
rehabilitation, case management and support service delivered by a multi-disciplinary team to individuals
who are diagnosed with severe persistent psychiatric disorders whose needs have not been met by
traditional services.

The Psychiatric Nurse Practitioner (PNP) will work as part of a multidisciplinary team providing coordinated treatment. The PNP engages the consumer in the shared decision making about medication and the next steps in medication treatment. Assessments of mental status, continuity of care and compliance of clients receiving services, instruction of psychopharmacology and mental health practices to clients and families, consultation with physicians and use of health care resources, and access for concomitant chemical dependency disorders are included in job summary.

RESPONSIBILITIES:

  • Assess the need for medications targeting specific symptoms of psychosis and other co-morbid conditions such as depressed mood and anxiety.
  • Systematically manage and monitor dose changes and side effects in order to ensure the safety and tolerability of these medications.
  • Will provide ongoing diagnostic assessment.
  • Will provide psycho-education regarding diagnosis and medications for participants and family members.
  • Will coordinate with clinical team to ensure quality care.
  • Works with the Program Director to monitor the clinical status and treatment response of each served individual.
  • Conducts assessments which include psychiatric history, illness course, treatment response, MSE, and arrive at I through V diagnoses and share results with team.
  • Directs psychopharmacologic and medical treatment and collaborates with the team nurse in assessing client physical health.
  • Assists in providing education to clients regarding their illness, response to treatment, and possible side effects of medications.
  • Oversees and signs treatment plans and participates in treatment planning process.
  • Ensures appropriate coordination of care with client PMD's.
  • Makes recommendations for specialty care.
  • With other team members, provides support, education, and counseling to family members to help in the understanding of mental illness.
  • Provides management and/or consultation in crisis situations.
  • May be required to submit written reports and/or verbal testimony and presentation of opinion in legal proceedings.
  • Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.

QUALIFICATIONS:

  • NYS licensed psychiatrist/NP
  • Experience in community-based mental health services.
