Case Manager of ACT Renew

Starting Rate: $22.60 per hour Full Time

ACT (Assertive Community Treatment) is an integrated evidence-based treatment,
rehabilitation, case management and support service delivered by a multi-disciplinary team to individuals
who are diagnosed with severe persistent psychiatric disorders whose needs have not been met by
traditional services.

SUMMARY OF DUTIES:

The ACT Case Manager has responsibility for carrying out rehabilitation and support functions and assists in treatment, substance abuse services, education, support and consultation to families, and crisis intervention under the clinical supervision of staff with professional degrees. The ACT Case Manager provides case management for assigned consumers, including coordination and monitoring of the activities of the individual treatment team.

RESPONSIBILITIES:

  • Coordination and primary implementation of case management activities.
  • Consults with the Program Director and psychiatrist regarding response to treatment interventions.
  • Provides activities of daily living skills training and mobility training
  • Provides money management and budgeting training.
  • Participates in Morning Meetings.
  • Provides program consultation and education to community members.
  • Acts as a liaison where a brokered, specialty service may be required.
  • Documents recipient progress as per established guidelines.
  • Responsible for the management of client charts.
  • Develops initial and on-going treatment plans.
  • When indicated, transport clients to scheduled activities.
  • Work with clients in their own natural environment.
  • Assist clients in accessing supports to maintain community stability.
  • Adhere to person-centered planning philosophy and service provision.

  • Accompany clients to scheduled court appearances.
  • Works with minimal supervision in the field.
  • Routinely conducts lethality and mental status assessments.
  • Participates in 24-hour, on-call rotation.
  • Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.

REQUIRED KNOWLEDGE AND SKILLS:

  • Has working knowledge of community resources.
  • Able to recognize and manage client crises.
  • Demonstrates creativity in service delivery and planning.

QUALIFICATIONS:

  • A bachelor's degree in a human service field and at least two years' experience as a case manager, working directly with severely and persistently mentally-ill individuals.

AND

  • A valid NYS drivers' license.
  • Automobile in good working order.
  • Must have Accredited NYS Defensive driving course; or obtain within 15 days of hire.
  • Must maintain Active NYS Defensive Driving class every 3 years, upon expiration.
  • If driving one's own vehicle, must maintain valid inspection and automobile insurance.
  • Must provide proof of insurance upon hire and when requested.
