Job Title: Intake Specialist - Housing

Reports To: P.D of Program

FLSA Status: Non-Exempt

Notice: 4 Weeks

Grade: 6


SUMMARY OF DUTIES:

The Intake Coordinator provides screening, assessing, and care coordination service to applicants applying for housing services. She/he maintains client records in accordance with agency and regulatory standards.


RESPONSIBILITIES:

? Conducts telephone and in-person screening, intake and assessment activities.

? Identifies and obtains documentation to determine eligibility for qualified housing program.

? Collaborates and communicates effectively with referral sources.

? Manages a prioritized waiting list for admission.

? Provides diagnostic impressions using the DSM 5.

? Works with interdisciplinary team to identify pre-housing needs for participants.

? Coordinate consents and contacts with prior, present, and future provides in order to promote continuity of care

? Participate in team case conferences

? Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.


REQUIRED KNOWLEDGE AND SKILLS:

? Must have knowledge of Substance Use Disorders, Mental Health Disorders, and community resources.

? Must be able to effectively relate to diverse cultural and economic backgrounds and to work effectively in a community setting.

? Must possess excellent oral and written communication skills.

? Must be able to build collaborative working relationships with community referral sources.

? Ability to speak Spanish is desirable.


QUALIFICATIONS:

? Master degree, QHP status, and one year experience in substance abuse or mental health program.

OR

? Bachelor's degree in social work, psychology, or closely related human service field,

PLUS two years full time paid direct service experience in an acceptable social services agency,

OR

? Two years in a substance use service program and current CASAC certification NYS

AND

? A valid NYS drivers' license.

? Automobile in good working order.

? Must have Accredited NYS Defensive driving course; or obtain within 15 days of hire.

? Must maintain Active NYS Defensive Driving class every 3 years, upon expiration.
